This video on Addressing Suicidal Thoughts and Behaviors in Substance Abuse Treatment was developed by the Department of Veterans Affairs, in collaboration with SAMHSA. The video adapted the contents from the SAMHSA TIP 50, Addressing Suicidal Thoughts and Behaviors in Substance Abuse Treatment, and is reformatted as a didactic tool for staff who work with persons with suicidal behaviors. A moderator and three member panel discuss ways to manage and address suicidal thoughts and behaviors among individuals with substance abuse issues. Case scenarios are presented and discussed by the panelists. This video provides with necessary information on “what”,” why”, and “how” one should treat clients with substance abuse issues and suicidal thoughts and behaviors. It provides information on suicide and substance use disorders, including risk factors, warning signs, and follow-up care. The counseling sessions portrayed in the video employ the specific counseling techniques and the four-step process recommended by TIP 50. Front-line supervision

Front-line supervisors spend more time with the workforce than any other level of management. They interface directly, on the job with employees. The main task of supervisors is to ensure that the work gets done by those reporting to them. They are held responsible for the productivity and behaviour of the employees reporting to them. Therefore, any action that affects these employees’ productivity and behaviour is of concern to the supervisor.

Drug and alcohol abuse/misuse is an increasing problem in Australia. In 1998 the cost to the economy was estimated at billion and by 2004 the cost estimate was up to billion. (Lauder, 2008). These costs are related to sickness, premature death, reduced productivity, crime and accidents. Drug and alcohol abuse/misuse affects the productivity not only of the employees themselves, but also of other employees.

Importance of training in drug and alcohol abuse/misuse for supervisors

In order to be effective in detecting drug and alcohol abuse/misuse supervisors need to be adequately trained. This training should include:

Company policy on drug and alcohol abuse/misuse
What is drug and alcohol abuse/misuse
Effects of drug and alcohol abuse/misuse in the workplace
How to detect drug and alcohol abuse/misuse
Company policy and practice in drug and alcohol testing

The role of drug and alcohol testing in the workplace

“Employee drug and alcohol testing is intended to ensure workplace safety, security and productivity.”

Employees who are under the influence of drugs and alcohol are a danger in the workplace; not only to themselves, but also to their co-workers. It is, therefore, important to test workers regularly. It is also important that this testing is done within the confines of the law and your company policy. You cannot just ask an employee to take a breathalyser test or collect a urine or saliva sample without several things being in place first. To learn how to collect samples correctly, register for the Workplace Based Certified Drug & Alcohol Collector – Nationally Accredited Training at CMM Technology. This training covers the following Australian Standards:

AS4308:2008 (Urine Drug Testing)
AS4760:2006 (Saliva Drug Testing)
AS3547 (Breath Alcohol Testing)

Guidelines for drug and alcohol abuse detection by supervisors

Supervisors need to look out for the following signs of drug and alcohol abuse/misuse:

Missed appointments, sick days, drunkenness or “on a high” on the job.
Relatives phoning in to explain why the employee is not at work.
Unexplained absences from work station.
Lack of tolerance of co-workers.
Irritability, restlessness, shakiness, trembling, sweating.
Regularly complaining of headaches.
Unexplained fatigue.
Not eating at lunch breaks.
Bloodshot eyes.
Pupils larger or smaller than usual.
Sudden weight loss.
Slurred speech.
Coordination problems.
Lack of motivation.
Seems paranoid for no reason.

If the supervisor suspects drug or alcohol misuse, the Drug and Alcohol policy of the company must be followed.

Conclusion

Drug and alcohol abuse/misuse can be a serious problem in the workplace and the front-line supervisor is in the ideal position to detect abuse/misuse. If equipped with the correct knowledge and skills the supervisor can detect drug and alcohol abuse/misuse at an early stage and can refer the employee for testing, counselling and treatment.

A person may use drugs for specific reasons. For example, my dentist might prescribe vicodin for pain after surgery. If I keep taking the drug after I have healed and can’t seem to stop using the vicodin, I’m abusing the drug. I’ve lapsed from drug USE into drug ABUSE.

The unfortunate synergy between criminal activity and drug use persists as a major challenge to public health and public safety systems. Today, half of the 2 million people incarcerated in state and federal prisons meet the criteria for drug dependence or abuse as described in the most recent edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) (Mumola and Karberg, 2006; Belenko and Peugh, 2005). Although the prevalence of drug dependence or abuse is highest among offenders committing non-violent drug-related crimes, an estimated 25% of state and federal prisoners report committing violent offenses while being under the influence of drugs. Violent crimes include such offenses as rape/sexual assault, aggravated assault, and murder, which represent the most costly of all crimes with an estimated cost per offense ranging between 7000 to over million (Rajkumar and French, 1997; Miller, Cohen, and Rossman, 1993). The annual growth rate in incarceration has slowed in recent years, but expenditures for criminal justice services (police, courts, corrections) have increased (Bureau of Justice Statistics [BJS], 2004). Blood Disorders: Induced by Drug Use & Abuse

Blood is an opaque, mobile fluid connective tissue that is mesodermal in origin. It consists of plasma, blood cells and platelets amongst a diverse array of molecules and ions. A large number of drugs, administered for treatment of various disorders come in direct contact with blood whether taken orally or injected intravenously. A multitude of possible drug interactions pose a serious problem especially to people who take prescription medicines for various diseases simultaneously. Some of these drugs may cause adverse effects on blood cells and lead to various blood disorders. Blood disorders are rare, yet extremely serious and may even prove fatal.

Types of Drug induced blood disorders: In our body, red bone marrow contains pluripotent stem cells which differentiate and mature to form various types of blood cells. Different drugs show different effects on various cells at different stages of cell development. Blood disorders can be characterized depending upon where and at what point in the cellular development the drug acts upon. Some of the common blood disorders include, but are not limited to:

Aplastic anemia: It is a form of anemia characterized by an abnormal deficiency in all blood cells resulting from failure of the bone marrow. It is a rare disorder of haemopoietic stem cells which was first found to be caused by the use of Chloramphenicol. Various non-steroidal anti-inflammatory drugs, disease modifying anti-rheumatic drugs have also been found to cause aplastic anemia.

Agranulocytosis: It is an acute blood disorder which is characterized by severe reduction in granulocytes and neutrophils. It is often caused by radiation and chemotherapy which are known to cause reduction in granulocytes and neutrophil counts. Some anti-thyroid drugs like carbimazole & propylthiouracil also increase the risk of Agranulocytosis.

Thrombocytopenia: It is a blood disease characterized by an abnormally small number of platelets in the blood. Heparin is one of the well known drugs associated with severe reduction in platelet count.

Hypertension: It is a common disorder in which blood pressure remains abnormally high (a reading of 140/90 mm Hg or greater). It is also called as high blood pressure. Amphetamines, corticosteroids, estrogens are some of the drugs that can lead to hypertension.

Symptoms:

The symptoms of drug induced blood disorders vary depending upon the type of drug used and the drug effects they produce. However, some generalized symptoms of various common blood disorders may include bleeding gums, chest pain, dry cough, dyspnoea (difficult or labored respiration), fatigue, malaise, pallor (unnatural lack of color in the skin), stomatitis (Inflammation of the mucous membrane of the mouth), lassitude (weakness characterized by a lack of energy), lymphadenopathy (Chronic abnormal enlargement of the lymph nodes), petechiae i.e. minute red or purple spot on the surface of the skin as the result of tiny hemorrhages of blood vessels in the skin, to name a few.

Treatment:

Treatment of blood disorders requires special expertise. Aplastic anemia may require Immunosuppressive therapy and bone marrow/stem cell transplant whereas Corticosteroid therapy is beneficial in case of Hemolytic anemia. As an immediate measure, use of suspected drug should be discontinued and short term supportive treatments should be given to the patients.
